Abstract Actinic keratoses (AK) are common in immunocompromised patients and associated with increased risk of cutaneous squamous cell carcinoma (cSCC). The current standard care is 5-fluorouracil cream 5% (5-FU) monotherapy for 4 weeks. In recent years, 5-FU calcipotriol ointment (5-FU-Cal) combination AK therapy 4–6 days has emerged as potentially more effective better tolerated, although the original randomized controlled trial (RCT) excluded immunosuppressed patients.
